Incident Report Policy

It is the policy of the Board of Trustees of the Lake Placid Public Library to create a 

written account of all incidents such as disruptive behavior or vandalism on Library 

property. 

Objectives 

The objective of this policy is to provide documentation that can be referenced to after 

an incident has occurred. 

Procedure 

When an incident takes place: 

  1. The Librarian-in-Charge shall be immediately summoned to the scene of the incident. 
  2. The Librarian-in-Charge and/or his/her designee shall remain at the scene of the incident through its conclusion. 
  3. The Librarian-in-Charge shall record the following facts to be included in a written incident report: 
    • The date and times that the incident began and ended.
    • The name, address, age, and telephone number of all persons involved in the incident (NOTE – where a name cannot be determined, a detailed physical description should be documented including the apparel worn by the person being described) 
    • The names of staff member who witnessed and or were parties to the incident 
    • The names and badge numbers/other identifiers of police and/or other persons responding to a 911 call, with their time of arrival and departure 
    • The number assigned to the police report 
    • Statements and claims made by persons involved in the incident 
  4. In instances of an un-witnessed incident claimed by a member of the public, a Librarian will create an incident report if requested to do so by a patron, noting that no witnesses were available to confirm the alleged particulars of the claimed incident. 
  5. In instances of an un-witnessed incident claimed by an employee, that employee with or without the assistance of his/her supervisor shall complete the incident report. 
  6. A staff member who completes an incident report shall provide the completed report to the Library Director as soon as possible after the incident takes place.
